Effective 10/30/2011, patients transferred to another program or facility will be required to take along a Universal Transfer Form. The form is to be prepared by the sending facility. OEMS has more information and the new form on its website.
In brief:
· The EMS/ambulance crew requests the form at the sending facility along with other medical documentation and then carries it to the receiving facility.
· If a facility does not have the form available, treatment and transport should not be delayed.
· Ambulance and EMS crews do not complete any information on the form.
· The Universal Transfer Form does not replace OEMS-required documentation.

The MTANJ is appreciative of Highmark Medicare Services' solicitation of feedback on its draft LCD about ambulance services. We received several comments from members and incorporated them into our response.
